At the bottom of this page is a link to a file that contains all of the National American Legion Baseball Forms # 1, 2, 76 & 77.   The file is in "ZIP" format.  If you do not have a "ZIP" program to "unzip" the files, then click here to download a free program that will assist you. After you download the file and extract the forms from the file, you need to click on the following  link so that you can download the Filler program, also in "ZIP" format, that allows you to fill out the forms on your local PC which, you can then print and save the information to your hard drive for future revisions.   FILLER PROGRAM  This software is free for your unlimited usage. These automated forms have just been designed for your use and will make your job easier when you submit the information to your State Baseball Chairman. The main advantage is that you can save the Forms to your hard drive and modify them without having to re-enter all of the information. These forms have been designed so that some of the fields on the Form are mandatory and will not allow you to printout the Form until data is entered. You can enter data on the Forms by either clicking on the field with your mouse or using the Tab key to move to the next field. After you down load the Forms and fill in the information, save the entered information by going to "FILE" and "SAVE AS", so that you can give the Form different name i.e. "Team_1", Waiver_1, Form2_Bob, Form2_Jim, etc. Reminder, you must download the software that allows you to  fill in the forms, also in "ZIP" format, and execute the installation file.  FILLER PROGRAM   Adobe PDF Format  ALB Form #1    ALB Form #2    ALB Form #76   ALB Form #77  To open Adobe PDF Forms     you will need Adobe Reader 5.0 or higher   Click here to download Adobe Reader Please note you can download these forms, however unless you own a copy of Adobe Acrobat 5.0 or higher you will not be able to save the data.
