If a Post needs the 'Certification of Service Record of American Legion Post Officers' form, they can use the .pdf version found here on the Department Website. It requires signatures by Post Adjutant and Post Commander. FAX or mail via USPS the completed form to Department HQ. Please retain a copy for your Post. Also send a copy to your District, and County Council (if required), etc... Date of enlistment, date of discharge and Service Branch IS NOT required for Department HQ record.
